Before practicing with any Hamilton athletic team, each student-athlete must provide the sports medicine department with information (note: this applies to all intercollegiate teams, as well as, some club teams). To expedite this process, Hamilton has incorporated an online data entry system. Â

Returning Users: log on by using the Athlete ID you previously selected and your previously selected password.
The following mandatory tabs must be completed:
1. General: Please use your Hamilton Email (without "@hamilton.edu") to make your Athlete ID
2. Insurance
3. Contacts
4. Forms: There are nine forms; all forms relevant to your sport must be completed
Medical History: Every question on the five pages must be answered (yes or no) and dates and/or comments must be provided for all yes answers. First time users, please complete this form with a parent and/or guardian.
In addition to sports medicine paperwork, all student-athletes must fill out NCAA compliance paperwork during a preseason compliance meeting.
